Reality Crossover Wild Magic for Fifth Edition

Wild magic tables are common. A private die is not. Attunement has each player roll d100 exactly once, at the start of a campaign, to learn which four numbers on a d20 are theirs and theirs alone. From then on, whenever an attack roll or a saving throw lands in that range, reality bleeds through. That single unrepeatable roll turns an ordinary d20 into something a character watches differently for the rest of the game, a number to want and a number to dread that happen to be the same number.

What comes through is a hundred-entry table of effects crossing in from somewhere they should not, running from harmless to genuinely uncanny, several with a foothold that outlasts the encounter that produced them. An escalation track keeps a character who triggers the Bleed again and again from reading as merely unlucky. Residue rules cover the results that do not fully leave. Reality Anchors give a party something to push back with.
